[data-issues] polarity reversal for XM.TX06

the network operators (Steve Grand and Dave Wilson) did confirm that this
site was the only site in the RISTRA line that had a polarity problem, 
and are
looking at notes to provide the time window when this occurred.
 There will be an update and a DPR filed when the details are determined.
